MedsCheck

The MedsCheck Ontario program is a medication consultation initiative offered by the Ontario Government free of charge to qualifying patients that aims to improve and simplify complicated medication therapy and improve overall health.

The program consists of regular consultations between you and a pharmacist and will include a thorough evaluation of your medication profile as well as your level of satisfaction with your particular therapy.  In addition, the notes taken and recommendations made are shared with your doctor so that your medication therapy is improved and any issues are resolved quickly.

MedsCheck Ontario Evaluations

Qualifying MedsCheck Consultations are funded by the Ontario Government and are provided by your pharmacist with no cost to you. Please speak to us at the pharmacy if you would like more information.

If you do not qualify for any of the MedsCheck Ontario evaluations listed below and would like a medication review, your private insurance plan may cover the cost of the consultation.

Annual MedsCheck

Available to patients who are taking 3 or more chronic prescription medications.

Annual In-Home MedsCheck

Available to patients who are taking 3 or more chronic prescription medications and are unable to travel to the pharmacy for their appointment due to physical limitations. In this case the pharmacist will visit the patient’s home and perform the MedsCheck consultation there.

Annual Long-Term Care MedsCheck

Available to patients who are living in a licensed Long Term Care facility regardless of the number of prescription medications being used.

Quarterly Long-Term Care MedsCheck

 Available to patients who are living in a licensed Long Term Care facility regardless of the number of prescription medications being used.

Annual Diabetes MedsCheck

Available to patients who are living with diabetes regardless of the number of prescription medications being used.  This consultation may include additional information on device use, lifestyle changes, and disease state management in order to improve the quality of life of our patients living with diabetes.

MedsCheck Followup

Available to patients who are taking 3 or more chronic prescription medications and require an additional consultation within the same 12 month period as their Annual MedsCheck.

Certain additional criteria must be met such as planned hospital visits or discharges, physician referral, or pharmacist decision based on significant medication profile changes.
